How to Make a Reservation
Please contact City Hall (320) 274-3055 to reserve the Park Pavilion.
The reservation form below and reservation deposit (50% of the reservation fee) must be submitted to the City within 5 days of the approved request to keep your reservation.
How to make a Payment
The City is able to accept cash, check or credit card for the reservation deposit. When submitting the reservation form, please indicate how you intend to make payment for your reservation.
Payments can be made using the following options:
- Online: Make a Payment Online
- Over the Phone: (320) 274-3055
- Drop off at City Hall: 30 Cedar Street East
- By Mail: PO Box K, Annandale, MN 55302

The reservation form below is an Adobe fillable form that works best when using Chrome web browser. If you are unable to use the fillable feature in your web browser or on your computer, you should be able to print the form and hand write the information.
